Abstract

Indonesia's high religious diversity places midwives at the intersection of clinical competence and religious sensitivity, yet systematic models for integrating religious respect into midwifery practice remain underdeveloped. This study aims to analyze the evidence base for religiously adaptive midwifery care in Indonesia through a systematic literature review, identify barriers to implementation, and propose a practical model grounded in Islamic moderation (wasatiyyah) and multicultural competence principles. Using thematic analysis, 18 studies were selected from 168 records identified across five databases (Google Scholar, PubMed, ScienceDirect, DOAJ, Garuda/Sinta) following PRISMA 2020 protocol. Four major themes emerged: (1) the nature of religiously competent midwifery, encompassing respect for modesty, halal-haram principles, prayer facilitation, and family-religious leader involvement; (2) barriers including limited religious training, absence of explicit SOPs, and institutional inertia; (3) Islamic moderation as the theological foundation for an inclusive yet principled approach to religious diversity in midwifery; and (4) a proposed adaptive midwifery service model with four pillars: multicultural-religious competence development, explicit SOP formulation, family-religious leader engagement, and personalized care. Findings from synthesized literature consistently show that religiously sensitive midwifery strengthens therapeutic relationships, improves patient compliance, and contributes to better maternal-child health outcomes. The article recommends integrating these principles into national midwifery competency standards and education curricula.
