Abstract

The intrinsic motivation of midwives particularly in remote and underserved areas of Indonesia is increasingly recognized as a critical determinant of service quality and professional sustainability. Religious values, particularly the Islamic conception of human dignity and the concept of khalifah (vicegerency), offer a profound and durable source of intrinsic motivation that extends beyond material incentives. This systematic literature review aims to analyze how the Islamic understanding of human nobility, supported by comparative perspectives from Christianity, Hinduism, and Buddhism, functions as a foundation for midwifery professional dedication. Using PRISMA 2020 protocol, 27 studies were selected from 186 records across five databases (Google Scholar, PubMed, Scopus, ScienceDirect, Garuda/SINTA). Five major themes emerged: (1) the theological architecture of human dignity, with QS. Al-Baqarah: 30 establishing khalifah as both a call and responsibility that transforms midwifery from a job into an act of worship; (2) religiosity and intrinsic motivation empirically linked to superior performance especially in Islamic hospital settings; (3) spiritual presence as a factor significantly improving the quality of birth experiences; (4) spiritual care as a driver of midwife motivation even under high workload conditions; and (5) the imperative of integrating religious-humanistic values into midwifery education. A conceptual model of Islamic Human Dignity-Based Midwifery Motivation is proposed, comprising three dimensions: theological foundation (khalifah, karamah insaniyyah, ibadah), motivational mechanism (transendental motivation as spiritual multiplier), and practical manifestation (sincere care, holistic competence, sustained dedication). Findings consistently demonstrate that religious values function as a spiritual multiplier of professional motivation, producing commitment that persists beyond the reach of external incentives.
